What Treatment Service is Best For Me?

During your assessment, a licensed counselor will collaborate with you to determine the most suitable treatment services for your recovery journey. Here’s an overview of the services Create Inc. provides:

Intensive Outpatient Treatment Services

The intensive outpatient treatment program (ASAM Level 2.1) focuses on client-centered care, addressing mental health diagnoses and substance use disorders through a combination of group therapy and individual sessions. Licensed alcohol and drug counselors and mental health providers will work with you to create a personalized treatment plan that supports your mental well-being and helps you develop healthy coping skills.

Services provided during intensive outpatient treatment include:

  • Client-centered and individualized treatment planning
  • Individual counseling sessions
  • Group counseling sessions
  • Diagnostic assessment and individual therapy
  • Treatment coordination

Group programming is three days a week and will meet in person or via telehealth, with options for day or evening sessions. Sessions are offered Monday through Thursday, 5:45 PM–9:00 PM or 9:00 AM–12:15 PM.

Relapse Prevention Programming

Relapse prevention programming (ASAM Level 1.0) is geared towards helping individuals who need ongoing support to maintain their recovery goals but do not require intensive outpatient or residential/inpatient services. These services will assist you with learning and implementing new coping skills, beliefs, and values to help you achieve your goals and maintain lasting changes.

Services provided during relapse prevention programming include:

  • Client-centered and individualized treatment planning
  • Individual counseling sessions
  • Group counseling sessions
  • Diagnostic assessment and individual therapy
  • Treatment coordination

Group programming is one day a week and will meet in person or via telehealth, with options for day or evening sessions. Sessions are offered Tuesday, Wednesday, and Thursday, 5:45 PM–9:00 PM or 9:00 AM–12:15 PM.

OPTIONS

OPTIONS (ASAM Level 1.0) is a 6-session program designed to help you assess personal consequences of substance use and identify behaviors to change to avoid future consequences. This program includes group and individual sessions over a two-week period via telehealth in the evenings. You will leave with a continuum of care plan to support your ongoing recovery.

OPTIONS is offered monthly.

Co-Occurring Mental Health Services

Integrated care is a key part of our approach. In all programs, CREATE provides comprehensive mental health services to address co-occurring mental health and substance use disorders. Co-occurring services include:

  • Psycho-social education groups
  • Mental health evaluations/diagnostic assessments
  • Individual therapy
  • Family education and therapy
  • Community referrals

Each service is delivered by qualified mental health provider to ensure holistic care. An initial diagnostic assessment will help to determine what specific services will best suit the client.
